Chapter 2 - The Emergency Room

The screaming wail of the ambulance siren faded into the background as the heavy automatic doors of St. Jude’s Medical Center burst open, welcoming a chaotic flood of fluorescent light and shouted medical codes. I was strapped tightly to a rigid spinal backboard, my head immobilized by foam blocks, my vision swimming in and out of a dark, murky fog of shock and blood loss. The paramedics who had rescued me off the nineteenth-floor canopy using a high-angle ladder apparatus were running alongside my stretcher, shouting vital signs to the trauma team waiting in Bay 4.

“Twenty-eight-year-old female, seven months pregnant, status post high-velocity fall from a second-story architectural structure!” the lead paramedic called out as they wheeled me into the trauma bay. “Sustained left clavicle fracture, probable pelvic contusions, and acute fetal distress! Blood pressure is dropping—ninety over fifty and falling! We have regular, high-frequency uterine contractions occurring every three minutes!”

I fought desperately against the heavy, suffocating wave of sedation the trauma nurse was preparing to inject into my IV line. I reached up with my uninjured right hand, my fingers wrapping around the scrub jacket of the attending trauma surgeon, Dr. Sarah Evans, with a grip fueled by pure maternal terror.

“Don't let him in,” I rasped, my voice barely above a dry, rattling whisper as blood from a laceration above my eyebrow dripped into my eye. “My husband... Dr. Marcus Sterling... don't let him near me... or my baby... he pushed me... he tried to kill us...”

Dr. Evans froze for a fraction of a second, her professional training instantly clashing with the chilling weight of my words. As the chief of trauma, she knew Marcus well—he was one of the hospital’s most celebrated orthopedic surgeons, a man whose name was engraved on donor plaques in the lobby. But when she looked down into my eyes and saw the raw, absolute truth burning through my physical agony, her expression hardened into a shield of clinical authority.

Before she could answer, the double swinging doors of the trauma bay flew open with a violent bang. Marcus strode into the room, wearing his white surgical coat over his evening attire, his face arranged into a masterful, Oscar-worthy portrait of a frantic, devastated husband.

“Clara!” Marcus shouted, pushing past a junior resident to reach the side of my stretcher. He reached out to grab my uninjured hand, his touch feeling like burning acid against my skin. “Sarah, thank God you're on duty! She stopped taking her prenatal antidepressants last week—she had a psychotic break on the balcony and threw herself over the railing! We need to get her up to the OR right now! I want to supervise the surgical intervention!”

“Get your hands off her, Marcus,” Dr. Evans said sharply, her voice cutting through the chaos of the trauma room like a scalpel. She stepped physically between Marcus and my stretcher, signaling for two burly hospital security orderlies to flank the bed. “You are not on duty, you are not the attending physician, and as the patient's spouse in a trauma situation involving domestic injury protocols, you are legally prohibited from being within five feet of this treatment bay.”

“Are you insane, Sarah? I am her husband! I am a senior partner at this hospital!” Marcus roared, his eyes flashing with a dangerous, predatory rage as he tried to push his way around the surgeon. “She is mentally incompetent! She is endangering my unborn child! I have the legal authority to direct her medical care!”

“Not anymore, Dr. Sterling,” a deep, resonant voice echoed from the entrance of the trauma bay.

I turned my head slightly on the rigid backboard to see a tall, broad-shouldered man in a dark trench coat step through the swinging doors, flanked by two uniformed city police officers. It was Vance, my private investigator. In his right hand, he held a digital audio player connected to a portable speaker.

“You lost your medical and legal rights five minutes ago when I handed the District Attorney’s office fifty-two high-definition cloud recordings of you and your mistress plotting to murder your wife for her trust fund,” Vance said coldly, his gaze locking onto Marcus like a predator sizing up trapped prey. “Now step away from the stretcher before these officers remove you from the building in handcuffs.”
